Pass criteria
- Every test vector returns the expected main fault.
- Every expected fault appears in the result set.
- Each fault includes module, location, severity, confidence, and repair protocol.
- Reviewer simulation returns a valid verdict.
- Schema and registry endpoints are reachable and versioned.
